David W. Prochot, 46, of Phoenix, AZ, passed away Wednesday, July 11, 2018.Cremation rites will be accorded. David was born November 4, 1971, in Decatur, IL, the son of Robert and Jane Ann (Cortelloni) Prochot. He worked at AT&T in Arizona for seven years. David loved baseball and was an avid Cubs fan.He is survived by his mother Jane Brisch of Decatur; cousin Kim Bretz and husband Mike of Casner, IL; cousin Chris Creamer of Peoria, IL; aunt Nita Creamer and husband Charlie of Cerro Gordo, IL.He was preceded in death by his father.
